Scheduling your first visit
- Call the office to schedule. If you're pregnant or planning a pregnancy, ask to establish care.
- Confirm with the office that your insurance plan is accepted.
- Arrive a little early with the items listed below.

What to bring
- A photo ID
- Your insurance card
- A list of the medicines and supplements you take
- Records from previous doctors, if you have them
- A referral, if your insurance plan requires one
